Catapult Health Check-up

Catapult Health Onsite Preventive Check-ups have returned to the fair! Schedule yours online.

Thank you for your interest! All available appointment slots have now been filled.

Catapult Health Check-ups are preventive health assessments that include lab work and tests for metrics like blood pressure, cholesterol, and glucose, followed by a virtual consultation with a nurse practitioner to review results and create an action plan.

Check-ups are free if you meet eligibility and have a PCP on file with BCBSTX HealthSelect. Appointments are required—have your HealthSelect ID card ready to register. You can change or cancel appointments anytime. For help, email support@catapulthealth.com.

  • Who is eligible:

    • All HealthSelect of Texas® participants who have a PCP on file, including those enrolled in Consumer Directed HealthSelectSM.
    • Age 18 and older living in the United States.
    • Have not had an annual preventive checkup with another provider in the last 12 months.

The participants listed below are not eligible and should visit their primary care provider (PCP) for any preventive services instead of scheduling an appointment at an on-site Catapult Health clinic:

    • Participants under the age of 18.
    • Participants living outside of the United States.
    • Women who are pregnant, have been pregnant within the past six weeks, or have a history of double mastectomy with bilateral lymph node removal.

    • Bring your  BCBSTX HealthSelect card and a valid driver’s license or state ID.
    • Fasting for 8-12 hours before your appointment is highly recommended to get the most accurate results. (If you are not able to do this, let the Catapult representative know in advance.)
    • Make sure you are well hydrated before you go as it helps with the blood draw.
    • If you are not able to do this, let the Catapult representative know in advance.
    • Take your medications as prescribed by your provider(s) the morning of your checkup.
    • Wear loose, comfortable clothing so you are able to bare your upper arm for your blood pressure measurement and blood sample.

You may also want to make a list of questions to discuss during your private consultation with the Catapult Health Nurse Practitioner.
